Avident Health LLC and Unite for HER have been awarded an independent quality improvement grant funded collaboratively by Pfizer Inc. and the National Comprehensive Cancer Network® (NCCN®). The grant supports a project that will deploy Avident Health's proprietary MyNavigator™ digital platform to enhance care delivery for patients diagnosed with metastatic breast cancer (mBC). This initiative is significant because it addresses critical gaps in ongoing care management, where patients often struggle to navigate complex clinical guidelines and access supportive services between clinic visits.
Metastatic breast cancer requires continuous, complex treatment management, with adherence, symptom monitoring, and whole-person supportive care being paramount. However, patients and their caregivers frequently face challenges in understanding clinical guidelines and connecting with community-based resources outside traditional appointments. The grant-funded project aims to bridge these communication gaps by delivering personalized, patient-friendly education aligned with NCCN Guidelines for Patients®, driving enrollment in Unite for HER's free supportive services, and equipping informal family caregivers to foster shared decision-making and treatment adherence.
The MyNavigator platform will serve as an interactive, home-based companion with several key features. It will deliver tailored education by translating complex clinical pathways into accessible, bite-sized modules tailored to each patient's treatment journey, empowering them to participate actively in informed decision-making. The platform will also streamline access to integrative supportive care by integrating Unite for HER's comprehensive services—including nutrition counseling, fresh food delivery, mental health counseling, and evidence-based integrative therapies—directly into daily navigation prompts. Additionally, it uniquely engages informal caregivers by including family members in a secure communication loop, ensuring they have the context, education, and early symptom alerting needed to support treatment adherence at home. Finally, it enables proactive care team interventions by capturing patient-reported check-ins and symptom trends to alert clinical teams early, preventing avoidable complications and reducing acute care utilization.
Dr. Maen Farha of Avident Health commented, "Navigating metastatic breast cancer is a daily reality that extends far beyond the four walls of the clinic. By combining the proactive digital coordination of MyNavigator with Unite for HER's exceptional integrative services and NCCN's gold-standard guidelines, we are creating a continuous circle of support that empowers patients, supports clinical teams, and actively engages the family caregivers who are vital to treatment success." Sue Weldon, Founder and CEO of Unite for HER, added, "Collaborating with Avident Health on the MyNavigator platform enables us to reach patients and their families directly at the exact moment they need guidance, removing barriers to our integrative nutrition, wellness, and emotional support resources."
The project will measure key quality improvement endpoints over the grant period, including improvements in patient knowledge concordance with NCCN Guidelines, rates of supportive service utilization, patient-reported decisional conflict, and therapy adherence metrics. This initiative matters because it represents a collaborative effort to leverage digital health technology and integrative services to improve outcomes and quality of life for metastatic breast cancer patients, potentially setting a model for comprehensive cancer care.
